INFORM YOUR SUPERVISOR
STEP TO FOLLOW IF YOU HAVE AN INCIDENT ON THE JOB:
- Report the incident and call 1-833-735-9262.
- Relyance Service Group Cliams / RIsk Management will direct all medical care.
- Life/limb threatening incidents, call 911 and then inform Relyance Service Group Claims/ Risk Management Team.
- Relyance Service Group Claims/ Risk Management Team will handle all correspondenece. questions and concerns after recieving the first report of injury.
- Full investigation of alleged incidents is required before acceptance of any incident as a claim
- Failure to follow all procedure may jeopardize coverage of your claim.
All employees are expected to:
- Comply with all health and safety policies and procedures.
- Report any unsafe conditions, hazards, or incidents promptly to their supervisor or the designated safety officer.
- Participate actively in safety training and awareness programs.
- Use personal protective equipment (PPE) as required.
- Take responsibility for their own safety and the safety of their colleagues.
- Cooperate with all safety-related investigations and audits.
How can you help?
- Report the existence of, or potential for, unsafe or unhealthful working conditions to the appropriate supervisor
- Know and observe all safety rules, practices, and procedures
- Understand that you are responsible for your own safety and working safely is a condition of your employment
- In the event an work-related injury occurs, follow the instructions indicated on the wallet card
- Keep the wallet card on you at all times
What to do if you are injured on the job:
- If it is a life/limb threatening incident, immediately call 911
- Notify your job site supervisor immediately and call the RSG Claims/Risk Management Team
- A detailed report of the incident is needed for all incidents and will be provided to you for completion at time of alleged incident
- A full investigation of the incident by RSG is required before acceptance of an injury claim
- RSG expects you to return to your job immediately following treatment. A full medical release, along with mandatory post-incident drug & alcohol test documentation is required
- Only the RSG Claims/Risk Management Team can authorize you to return to work. Your supervisor, lead, or even on-site manager cannot authorize your return to work
- PLEASE NOTE: Failure to follow these procedures may jeopardize coverage of your claim
